Greenfield Rehabilitation Agency is thrilled to expand our leadership team and is seeking a dedicated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to serve as Rehab Director at our respected senior living community in La Crosse, WI .
This is an outstanding opportunity for a motivated and compassionate therapy professional who values clinical excellence, teamwork, and leadership . As Rehab Director, you will guide the rehabilitation department, mentor staff, and ensure residents receive the highest level of care , all while shaping a positive, therapist-driven work culture.

Lead and manage therapy department operations , ensuring collaboration and clinical excellence
Supervise, mentor, and inspire therapy staff, fostering growth and engagement
Oversee treatment planning, scheduling, and compliance with all state and federal regulations
Monitor therapy outcomes, documentation, and productivity metrics
Encourage innovative, patient-centered care and clinical problem-solving

Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) in Wisconsin
Leadership or supervisory experience in skilled nursing, rehabilitation, or senior care preferred
Excellent communication, organization, and mentoring skills
