Greenfield Rehabilitation Agency is thrilled to expand our leadership team and is seeking a dedicated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to serve as Rehab Director at our respected senior living community in La Crosse, WI .

As Rehab Director, you will guide the rehabilitation department, mentor staff, and ensure residents receive the highest level of care , all while shaping a positive, therapist-driven work culture.

Oversee treatment planning, scheduling, and compliance with all state and federal regulations
Monitor therapy outcomes, documentation, and productivity metrics
Encourage innovative, patient-centered care and clinical problem-solving

Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) in Wisconsin
Leadership or supervisory experience in skilled nursing, rehabilitation, or senior care preferred
Excellent communication, organization, and mentoring skills
